Savanna Healthcare is committed to protecting the privacy of the people we work with. This policy explains how we collect, use, store and disclose personal and health information, in line with the Privacy Act 1988 (Cth), the Australian Privacy Principles and the Health Records and Information Privacy Act 2002 (NSW).

What we collect

• Names, contact details and dates of birth
• NDIS numbers and plan details
• Health and disability information, and information about support needs, goals and living arrangements
• Contact details for referrers, guardians, plan nominees, support coordinators, plan managers and other providers involved in a person’s supports

How we collect it

We collect information directly from you wherever possible, including through our online referral form, by email, by phone and during assessments. With consent, we may also collect information from referrers, families, support coordinators and other providers involved in your supports.

Why we collect it

• To respond to referrals and provide occupational therapy assessments and reports
• To communicate with you and the people involved in your supports
• To invoice for our services, including through plan managers
• To meet our legal and professional obligations

Who we share it with

With your consent, we may share information with the NDIA, your plan manager, your support coordinator and other providers involved in your supports. We may also disclose information where we are required or authorised to by law. We do not sell personal information.

How we store and protect it

We take reasonable steps to protect personal and health information from misuse, loss and unauthorised access. Records are kept securely and retained for as long as required by law, then securely destroyed.

Information submitted through our online referral form is sent to us by email and stored temporarily on our website, where it is automatically deleted after 90 days.
